Based in Libertyville, Illinois, CarePartners is a specialty pharmacy platform providing home and ambulatory infusion services to patients who suffer from chronic conditions. CarePartners is licensed to serve patients in all 50 states and collaborates with insurers, drug manufacturers and physicians to deliver patients high-touch and cost-effective clinical services.
